ADULT FOSTER CARE (AFC) Overview
AFC is an alternative to nursing homes and assisted living placements. AFC in Massachusetts is a MassHealth funded program that provides support to qualified people with physical, mental, or cognitive challenges to live in their home or with a qualified caregiver.
Qualified Members (Those Receiving Care) Must:
- Be 16 years of age or older
- Be enrolled with MassHealth (Standard or Common), Commonwealth Care Alliance (CCA) – Senior Care Options, Fallon Community Health Plan – Senior Care Options, Wellsense Health Plan – Senior Care Options (formerly BMC), Molina Healthcare (formerly SWH), Tufts Health Plan – Senior Care Options, United Health Care – Senior Care Options.
- Has a medical, behavioral, or cognitive impairment that requires supervision and coaching to successfully complete at least one of the following activities provided by a live-in caregiver: bathing, dressing, toileting, transferring, mobility, eating, medication reminders, and scheduling
Qualified Caregiver (Those Providing Care) Must:
- Be at least 18 years old
- Live in the same household as the member (family or non-family member)
Amount – Up to $21,000 TAX FREE depending on the level of care.
AFC is a medical service that must be prescribed by the member’s PCP office.
